TERMS AND CONDITIONS
I/We agree and acknowledge:Only enrolled children, casual or full time, will be accepted into care.

To collect children from the Centre no later than 6.00pm every day (there is a late collection charge of $10.00 per child per 1/2 hour) and to advise if someone other than the appointee person will be collecting child/ren.

To pay fees within 7 days of receiving an invoice, or if casual, upon collection of the child/ren that day and to pay for late collections of child/ren on arrival. All outstanding fees must be paid in full at the end of each term.

All Internet payments must be paid in full at the end of each term.

To advise the Supervisor on each occasion a child will not be attending before 12pm that day or a penalty one hour fee may be charged. Any concerns regarding fees charged are to be addressed to the Head Supervisor.

To advise the Supervisor of any situation that may disturb that child/ren and/or any medical or other relevant conditions.

To ensure children know where to go and inform the school if they are casual or new attendees.

Behaviour which is consistently harmful to others may result in dismissal from the programme after all possibilities have been explored.

To immediately advise the Supervisor of any change to contact particulars on the enrolment form.

That the Supervisor may arrange any necessary urgent medical treatment at my/our cost.

While the management of the Pukerua Bay After School Care Group (Inc), its employees will take care to provide proper supervision of all children, neither the management nor the staff shall have any personal liability in respect of any act of omission arising in any session or activity of the child care programme.
